A diagnosis summary, pathology report, recent imaging reports and original image files, laboratory or molecular test results, previous treatment records and a current medication list are useful. Additional material may be requested for a specific diagnosis.
Is an initial record review a diagnosis or treatment plan?
No. It helps check whether the file is complete and identifies the relevant clinical department. Diagnosis and treatment decisions require specialist assessment and may require an in-person consultation or further examinations.
